This website collects only the information needed to process an appointment request, such as the patient’s name, phone number, optional email, appointment choice and a brief optional reason for visit.
How information is used
Booking information is used by the clinic to schedule, confirm, change and manage appointments and to contact the patient about the requested visit. Appointment details are available only through the password-protected administrator area and the hosting database.
Please avoid sensitive details
The public booking form is not intended for detailed medical records, prescriptions, reports or emergency information. Keep the optional reason for visit brief. Share clinical documents directly with the doctor or clinic using an appropriate method.
Retention and requests
Appointment records may be retained for clinic administration and lawful record-keeping needs. Patients may contact the clinic to request correction or deletion where appropriate and legally permitted.
